2. An estimate of how large you'd like your tattoo to be in inches

3. Where on your body you'd like to get tattooed

4. If you want a color or black and grey tattoo

5. Any special needs, requests, or allergies that I should know about (the main allergy concerns are food, chemicals, pigments, adhesives or latex)
